Step 2:  Mail your payment.
 
- Make cheques or money order payable to:  Aldershot Lions Club

- Send your fees (including your Team's name) to:

 - Payment is required before January 30th 2009

            For 5 players (5x$70) = $350
            For 6 players (6x$70) =$420


Step 3:  Sign the waver form.

Please ensure that each team member brings a signed waiver to the Challenge (February 7th 2009).



Step 4:  Confirmation.


Once this form is submitted (see “submit button” above) we will acknowledge your registration and advise you (the Team Captain) when we have received payment.

IMPORTANT!  Alternate Event:

If there is no ice, or conditions are deemed unsafe for Pond Hockey, a Road Hockey Challenge will be played in the parking lot.


Note:       If a team chooses not to participate in the Road Hockey Challenge, 50% of the team’s registration fees will be returned by cheque via mail to the team Captian.
