NOTE: To register a group less than four (including a single player) simply leave the remainig golfer name's blank. Ensure one player (or in the case of a single player) is designated as the Team Captain. For example, if 2 players are registering have one player register as the Team Captain. In the case of a single player registering, please register as the Team Captain.
